Ikego Enrollment
CFAY and Yokosuka Complex schools have established specific school assignments for children. Only residents of Ikego Heights Housing are authorized to attend Ikego Elementary School. Kindergarten to third grade students residing in the Ikego Housing area are to be enrolled at Ikego Elementary school. Fourth and Fifth grade students residing in Ikego Housing area are to be enrolled at The Sullivan’s Elementary School.
Enrollment at Ikego will be accepted only upon verification of acceptance of quarters on Ikego Base. Families in transit at the Navy Lodge or other Navy billets who accept Ikego Housing but have enrolled at The Sullivans’ Elementary School must report to Ikego Elementary School to update registration information prior to the first day of attendance. All documentation must be completed by 1000 prior to the first day of attendance.
Exceptions to this policy are entertained based on criteria, including student space availability, medically and/or educationally related services, and transportation costs. Approval for exceptions will be from both schools’ principals and CFAY Schools’ Officer. Initial requests should be directed to the “losing” principal.

print ALL of the documents below. Fill them in and bring them to
school when registering your child. You will need Adobe Acrobate Reader
to view these documents. All forms are also available in the school office
for your convenience.

Returning Students
All returning students
must present verification of current immunizations. You may bring DoD
immunization verification form DSJN 122b which may be obtained at the
immunization clinic, Yellow Shot Card, or Copies of child's medical
record for verification of immunizations.
As of SY 2007-2008, ALL children must have:
* LAST DOSE SHOULD BE AFTER AGE 4
* DPT - a total of 5
* OPV - a total of 4
* MMR - a total of 2
* Hepatitis B series- 3 out of 3 shots (or currently in progress, but
up to date)
* PPD with in 3 years of Nov. 1.
* VARIVAX All children aged 10 and above must have vaccine if they have
NOT had chicken pox.
* Hib (Haemophilus influenza type b) series
NOTE: Any students without verification of all immunizations will
not be allowed to register or attend school until appropriate documentation
is provided.
New Students must also submit the following documents
Student's Passport or a copy of Birth Certificate
Student's Social Security Number
At school, pick-up registration packet and complete:
____DSPA Form 2030.2 student eligibility
____DoDEA Form 600 student registration
____DoDEA Form 600A Questionnaire for Race/Ethnicity, and Home Language
____Request for Student Records from previous school (except New Kinder)
____Parental permission for on base school activity
____Special need form
____School Health Record ( report any MEDICAL condition that your child
has been seen by the Doctor for on an ongoing basis)
____Internet Policy form (New forms that all students will need to have
signed for 2002-2003 School Year)
____Information Center form
____Volunteer Form (for parents that wish to volunteer at the school)
All current Third Grade students
will register for the fourth grade at The Sullivans. All student files
and medical records will be transferred to The Sullivans school at that
time.
